Some problems that are perceived as “behavioral” actually turn out to be medical.  For this reason, a physical exam and potential lab work is needed
to effectively diagnose your pet’s problem.  Your referring veterinarian can perform the physical exam prior to the behavioral consult.

Before your appointment:
Your first step is to completely fill out the behavioral history form.  This will give us a lot of critical information to effectively evaluate the current issues.  
Canine behavior history form (electronic form): Answers can be typed and printed or emailed to vets@milwoodanimalclinic.com.
Canine behavior history form (manual form): Answers must be hand written can be dropped off or faxed.  
Feline behavior history form (manual form):   Same as above, for cats.
Feline inappropriate elimination form (manual form):  Use this form if your cat is urinating or defecating outside his or her litter box.

We also recommend you videotape the specific behavior problems occurring in the natural context.   **If your dog is exhibiting aggression, please do
not encourage or in any way try to induce the problem behavior.  Do not risk injury to yourself or others in order to get the video.

You should also review our
Canine Behavior Management handout concepts to institute while you are waiting for the consultation.  These are not
solutions to specific problems, but will help to provide consistency and predictability for your dog.  If you have a feline behavior issue a management
handout will be sent to you based on your cats specific needs.

Scheduling the appointment:   Call our office at 342-9865 to make an appointment for your pet’s Behavioral Consultation.   Please turn in your
Behavioral History Form at least 24 hours prior to your appointment to allow Dr. Updike to review it in advance.    

At Your Appointment:
Please bring all medication, collars, leashes, and training aids you have used with your pet.  Don’t forget to bring any video footage you have
collected.  We have televisions in our consultation rooms where you can plug in video cameras with camcorder patch cords.  
Your behavior consultation will take approximately 1-2 hours.  Dr. Updike will take a detailed history.  At that time, lab work may be recommended if
indicated.  After that point, a diagnosis will be made and we will work together to develop a treatment plan specifically for your pet.  

Follow Up:
Most case require follow-up behavioral modification sessions after the behavior consultation to work on your pet's specific problem behaviors.  Kari
Stasienko can work with you in hour home or in our clinic to provide this training one-on-one.

Fees for Services:
The initial behavior consultation fee is $75 for cats and $95 for dogs for the 1-2 hour appointment.  In most cases, a health screen blood work will be
performed to assess liver and kidney function prior to starting behavioral medications, the cost for this panel is $59.20.  Occasionally, additional tests
will need to be performed an a detailed estimate will be provided for any additional fees.  The fee for private training sessions with Kari is $59/hour.
